Financial Aid Forms

Listed below are the most commonly used Financial Aid forms.

Please do not submit Loan Requests or any Verification info until Peninsula College has received your FAFSA for the Academic Year. Loan Requests cannot be processed until a student's FAFSA is received, file completed, reviewed, and an eligibility determination made.

Verification forms are required for students whose FAFSA was selected by the Department of Education* or the school for a review process called verification, in which certain info reported on the FAFSA must be confirmed by the student and/or parent. “Dependent” forms are for students whose parental info was required on the FAFSA, who are under the age of 24; parent info and signatures are also required on the “Dependent” forms. “Independent” forms are for students who met the FAFSA’s criteria for being considered independent, meaning parental info was not required on their application.

All required forms will be made available in ctcLink through the Student Portal once their FAFSA is received by the college.

*The Department of Education requires the school to request certain information from the student (and parent, if the student is a Dependent) in order to verify information that was reported on the student's FAFSA. The school is not able to waive this requirement, and cannot disburse financial aid until all required forms/documents have been submitted and the verification process has been completed. The school may ask for additional clarification if there is conflicting or missing information.

How to Submit Forms

Completed forms may be mailed, delivered in person, or faxed to the Financial Aid Office. You can also use the secure Dropbox link.

**Please Note: Emailed forms which contain your personally identifiable information (known as PII) will not be accepted. Personally identifiable information includes your Social Security Number (SSN), birthdate, driver's license, tax return info, and any other data that could be used to verify your identity. Emailing this info is not a secure way of submitting it, so we politely but firmly request that you choose a more secure method of providing us with your info, such as the Dropbox link above.

Forms for 2024-25 Academic Year

The 2024-25 Academic Year begins in Summer 2024 and extends through Spring 2025, and relates to the 24-25 FAFSA.

• Program of Study Update Form
  This form is used to confirm or update a student's program of study.

Verification Forms:

• Dependent Standard Verification V1
• Independent Standard Verification V1
• Dependent Custom Verification V4
• Independent Custom Verification V4
• Dependent Aggregate Verification V5
• Independent Aggregate Verification V5
• Verification of Non-filing Statement
  This form is used for Independent students and parents of Dependent students who indicated they did not file a tax return for 2022. 
• Verification of Unaccompanied Homeless Youth
  For students who are under the age of 24 and experiencing homelessness

Loan Forms:

• Loan Request
  Entrance Counseling and Master Promissory Note must be completed prior to submission.
• Additional Loan Request
  For students who need to make adjustments to their currently awarded loans.

Appeal Forms:

• Appeal for Financial Aid Reinstatement
  This form is used to petition for financial aid reinstatement after a student's aid eligibility has been lost due to not meeting Satisfactory Academic Progress (SAP).
• Appeal for Time Limit Extension
  This form is used to petition for an extension of aid beyond the program limits.

Change in Financial Circumstances Form:

• Income Re-evaluation
  This form is used to document changes (decreases) in household income when a student or parent's current income is dramatically less than what was reported on the FAFSA. Students must have already submitted their 24-25 FAFSA, completed their financial aid file, and had their file reviewed for aid eligibility BEFORE they can submit this form.
