Peninsula College uses online forms which are accessible via Microsoft Forms. Students may access these via their myPC account.
After the student registers for classes, they will receive a welcome email for myPC which grants student's access to Canvas, Microsoft 365, a pencol student email account, and other online college resources.

- SAP Appeal (Appeal for Reinstatement)

This form is used to petition for financial aid reinstatement if your aid eligibility has been lost due to not meeting Satisfactory Academic Progress (SAP). SAP requires a 2.0+ cumulative GPA and the successful completion of at least 67% of all attempted credits. Supporting documentation for your appeal is required.
We are currently accepting SAP appeals for the Fall 2026 Quarter only. You must be enrolled in the quarter you are appealing for financial aid reinstatement.

- Time Limit Appeal

The Time Limit policy says you can receive financial aid for up to 150% of the credits required to complete your program; ALL previously attempted credits are counted. This form is used to petition for an extension of aid beyond that limit.
- Income Re-evaluation Request

This form is used to document changes (decreases) in household income when a student or parent's current income is significantly less than what was reported on the FAFSA. You need to have already submitted your current FAFSA or WASFA, submitted all required documents for your financial aid, and have your application initially reviewed for aid eligibility before you can submit this form.

IRS Verification of Non-Filing Letter (IRS Form 4506-T)
An IRS Verification of Non-filing Letter provides proof that the IRS has no record of a filed Form 1040 for a specific tax year. Note: If you typically would have filed a Puerto Rican or Foreign Income Tax Return you must submit appropriate non-filing documentation from a relevant tax authority. If you have trouble entering your street address into an online form, try these address matching suggestions.
Non Tax filers can request an IRS Verification of Non-filing Letter, free of charge, from the IRS in one of three ways:

Note: This is typically not available if you have never filed taxes before in prior years. If this is the case, please use the paper request process detailed below.
- Click “Get Transcript Online” (If at any point, you cannot validate your identity – for example, you cannot provide financial verification information or you lack access to a mobile phone – you may use Get Transcript by Mail, see below)
- Follow instructions for creating an account or log in. To create an account, you will need your Social Security Number or Individual Tax Identification Number, email address, birthdate, mailing address from latest tax return, tax filing status, account number for loan or credit card associated with your name, and mobile phone associated with your name.
- Select “Verification of Non-filing Letter” and in the Tax Year field, enter the tax year we are requesting. For example, you will enter tax year “2024” for the 2026-2027 school year.
- If successfully validated, you will be able to view your IRS Verification of Non-filing Letter.

- Non-filers can expect to receive a paper IRS Verification of Non-filing Letter at the address provided in their telephone request within 5 to 10 days from the time of the request.
- IRS Verification of Non-filing Letter requested by telephone cannot be sent directly to a third party by the IRS.

- Complete lines 1 – 4, following the instructions on page 2 of the form.
- Line 3: enter the non-filer’s street address and zip or postal code. Use the address currently on file with the IRS. Problems entering your street address?
- Line 5 provides non-filers with the option to have their IRS Verification of Non-filing Letter mailed directly to a third party by the IRS. Do not have your IRS Verification of Non-filing Letter sent directly to Peninsula College.
- Line 7: Select the checkbox on the right hand side for Verification of Non-filing.
- Line 9: Year or period requested field, enter the ending date of the year or period using the mm/dd/yyyy format. You will enter the tax year we are requesting. For example, you will enter “12/31/2024” for the 2026-2027 school year.
- The non-filer must sign and date the form and enter their telephone number. Only one signature is required when requesting a joint IRS Verification of Non-filing Letter.
- Mail or fax the completed IRS Form 4506-T to the address (or FAX number) provided on page 2 of Form 4506-T.
- If the 4506-T information is successfully validated, tax filers can expect to receive a paper IRS Verification of Non-filing Letter at the address provided on their request within 5 to 10 days.

Verification forms are required for students whose FAFSA was selected by the Department of Education* or the school for a review process called verification, in which certain info reported on the FAFSA must be confirmed by the student and/or parent. “Dependent” forms are for students under the age of 24 whose parental info was required on the FAFSA; parent info and signatures are also required on the “Dependent” forms. “Independent” forms are for students who met the FAFSA’s criteria for being considered independent, meaning parental info was not required on their application.
*If the Department of Education requires the school to request certain information from the student (and parent, if the student is a Dependent) in order to verify information that was reported on the student's FAFSA, the school is not able to waive this requirement, and cannot disburse financial aid until all required forms/documents have been submitted and the verification process has been completed. The school may ask for additional clarification if there is conflicting or missing information.
All required forms will be made available in ctcLink through the Student Portal once your FAFSA is received by the college.
Federal Direct Loan Forms
Please do not submit loan request forms until Peninsula College has received your FAFSA for the academic year. Loan requests cannot be processed until a student's FAFSA is received, file completed, reviewed, and an eligibility determination made.
